	Spring Valley Hospital Medical Center Spring Valley Hospital Medical Center is part of Universal Health Services (UHS), one of the nation's largest and most respected health care management companies. Spring Valley has partnered with CapMed to distribute 13,500 PHRs to at their annual health fair, to graduates of its neonatal intesive care program, and patients being discharged from the hospital. 	Helen F. Graham Cancer Center, part of the Christiana Care Health System Helen F. Graham Cancer Center, part of the Christiana Care Health System in Wilmington, DE will provide the HealthKeys to patients. The customization of the HealthKey includes the inclusion of Christiana Care's patient registration and medical history forms. When visiting the cancer center the HealthKey is 'plugged-in' to a staff member's computer to provide updated patient information. Additionally the HealthKey will allow patients to track and manage all facets of health information, and link to relevant health and wellness education and other Christiana Care Health System resources.
	Thomas Jefferson University Hospital Thomas Jefferson University Hospital in Philadelphia, Pennsylvania, part of Jefferson Health System, has partnered with CapMed to customize the Personal Health Manager. Customization of the application includes branding, patient registration forms, physician lists, and age-specific health guidelines. Thomas Jefferson is distributing 100,000 PHMs by mail throughout the community of Philadelphia and at request through its website.
	College of St. Scholastica The College of St. Scholastica in Duluth, Minnesota, is using the Personal HealthKey as part of the training for students in the Healthcare Informatics and Information Management Department. The St. Scholastica Personal HealthKey will be used to educate these future healthcare professionals about personal health records, the important role the patient plays in health information management, and how personal health records can help them to manage their own health. The Personal HealthKey includes direct links to St. Scholastica's student resources such as their health and wellness center.
	Pediatric Health Care at Newton-Wellesley Pediatric Health Care at Newton-Wellesley is committed to helping to keep its families healthy and to empower them to take charge of their health. Through its relationships with NextGen Health Care Information Systems, the practice’s electronic medical record software provider, and CapMed, Pediatric Health Care is providing the Personal HealthKey™ to help parents better manage the health of their children. The HealthKeys will be pre-populated with the child’s health records from the NextGen EMR system. The HealthKeys are also customized to the practice, and include valuable personalized educational content, as well as important medical and school forms.
