CapMed’s PHM enables individuals to become partners in their health care by providing tools and information to help them track their medical information on a personal computer. Ultimately this leads to improvements in patient and provider outcomes. The PHM is outfitted with Jefferson University Hospital’s patient registration and health history forms, their provider directory, and health resources and guidelines.

“We are very excited about our Personal Health Manager campaign,” said Carmhiel J. Brown, senior vice president of Marketing, Public Relations and Communications at Thomas Jefferson University Hospital. “At Jefferson University Hospital, we are dedicated to excellence in patient care. The interactive Personal Health Manager helps our patients take a more active role in their care. As an added benefit it also helps us to brand our Jefferson University Hospital name in the community.”

PHM will enhance Jefferson University Hospital’s brand through customized packaging, customized content that automatically builds a profile for users based on personal needs, insertion of logos on all reports, and promotional content included on the software startup screen,. In addition, the PHM software will directly connect users to Jefferson’s online resources, trigger branded reminders, and provide information on age-specific health management guidelines. Thomas Jefferson University Hospital is planning to launch its branded PHM by distributing it through the mail to the Delaware Valley community. The Hospital will also offer the PHM to individuals who request it in response to its advertising campaign, which will be carried out by local newspapers and prominently displayed on the Hospital’s website.
