CapMed provides interactive patient outreach solutions that enable patients to take an active role in their personal health. CapMed solutions are built on the CapMed Personal Health Management Platform™, a full-featured software program that assists patients in managing their personal health. The CapMed Personal Health Management suite of tools, driven by CapMed’s award-winning Personal Health Record (PHR), are interoperable, portable, and secure.

The CapMed PHR optimizes user engagement by offering three distinct, but interchangeable formats:

Personal HealthKey – portable PHR application, completely self-contained on a secure USB drive, enabling information to be shared and updated on any USB-enable computer
Personal Health Record – desktop PHR application, installed on the personal computer with a CD-ROM, supporting mass distribution and initializing user engagement
Online PHR – secure, online version of the PHR application, allowing all or part of the record to be managed and shared online

CapMed’s highly flexible, modular product allows for the ability to integrate with multiple other vendors and tools to offer a wide variety of valuable services to the PHR end-user. Product integration is completely driven by the requirements of the sponsoring organization. Organizations can incorporate tools already available or those offered by CapMed’s preferred partners. Modules can include forms generation, drug interaction checkers, gaps in care analysis, personalized patient content, and much more!

Designed for optimizing communication between patients and providers, the information stored by the end-user in the CapMed PHR can be shared in several ways: exporting through secure, server-based data exchange, online access granting to providers and/or caregivers, the portable Personal HealthKey, and printed reports. Exported data can be integrated with a variety of systems, including EMR and CRM systems, to support each organization’s interoperability goals. Additionally, CapMed’s PHR can import data from several sources, including claims information, EMR data, medication history, lab results, home monitoring device readings, and other electronic portable devices.

Once the software has been configured, the interface and packaging is customized to reflect your brand. The application can be branded with the sponsor’s look and feel throughout the entire application, which can even include the sponsor’s own content and tools.

In addition to its comprehensive solutions, CapMed offers complete technical and project management services and support to ensure a successful implementation and program launch. CapMed also offers custom development services to organizations who want to add unique or proprietary capabilities to the platform.
