ServicesCapMed offers clients a range of services to assist them in planning, implementing and supporting a successful interactive patient outreach program. PlanningNeeds AssessmentAt the beginning of each relationship, CapMed conducts a thorough needs assessment with each client. During this stage, CapMed works closely with the client to establish program goals, available resources and critical success factors. Program ProposalCapMed actively works with each client to establish a shared vision and scope for each program. In this phase, CapMed defines program details as well as estimated implementation timelines and client investment. ImplementationProject Kick-offOnce the program proposal has been accepted, the CapMed project team is formed and holds initial implementation meetings with appropriate client stakeholders. Solution SpecificationIn this phase, solutions and technical specifications are determined and documented. At the end of this phase, implementation timelines are set and necessary resources are allocated for the solution build. Solution BuildAfter the solution specification is approved, the CapMed team configures the client’s customized patient outreach program based on the agreed upon solution specification. Solution DeploymentThe final phase involves both integration and user acceptance testing in preparation for deployment to the end user. IntegrationCapMed plays an active role in creating technology standards for patient provider information exchange within the industry. CapMed has been active in interoperability showcases demonstrating connectivity with over 20 different EMR vendors using both the IHE exchange and the patient as the "chauffeur" with the Personal HealthKey following both the HL7 and CCR standards. CapMed has been showcased by both NextGen and GE Healthcare. The information stored by the end user in the CapMed Personal Health Platform can be exported through a secure, server-based data exchange, point-of-care-kiosks, and printed reports. The Personal HealthKey can be accessed using any computer with a USB port. The platform can be integrated with a variety of existing technology systems, including electronic records management (EMR) and customer relationship management (CRM) and home monitoring devices. SupportProgram SupportCapMed provides each client with a dedicated program manager to assist with solution deployment and maintenance. End-User Training and SupportCapMed offers a variety of training options to support end-user adoption. Interactive demonstrations, downloadable instruction manuals and FAQs, as well as software upgrades are available on the CapMed Web site. Technical support is also available by e-mail or telephone. |
